EPFO introduced new composite claim forms for full partial withdrawal. EPFO has launched Composite Claim Form for EPF withdrawal in death cases. The new Composite Claim Form – Death Cases is a replacement of Form 20, Form 5-IF and Form 10D.
EPFO also launched EPF composite claim form Aadhar and EPF composite claim form Non – Aadhar as a replacement of regular PF withdrawal forms like PF Form 19, PF Form 10 C and PF Form 31. Find the sample of how to Fill PF Composite claim forms for Aadhar and Non – Aadhar.
Earlier, In the death cases of member consists the process of submitting different forms to claim separately for EPF Form No. 20, for Insurance Form 5 – IF and for monthly pension (EPS) Form 10D by the nominee or legal heir of an employee.
EPF Form No.20
It is used by the nominee, legal heir or guardian of the minor/lunatic for claiming the Provident Fund accumulation of the deceased member.
EPF Form 5-IF
It is used by the nominee, legal heir or guardian of the minor/lunatic for claiming the Insurance (EDLI) of the deceased member.
EPF Form 10D
It is used by the nominee, legal heir or guardian of the minor/lunatic for claiming the monthly pension under EPS Scheme of the deceased member.
But recently The central provident fund commissioner prescribes composite claim form in death cases by replacing existing forms no. 20, 5-IF and 10-D. In the case of death of a member, the claimant may apply for the claim of provident fund, insurance fund and monthly pension in this single form.
A nominee or legal heir of deceased member has to provide following mandatory detail to fill the Composite Claim Forms – Death Cases.
- Name of deceased member
- Selection that Form is filled for PF withdrawal, EDLI or for Pension
- Marital status of deceased member
- UAN number or PF Number
- Particulars details of the claimant/Nominee
- Bank account detail in case of PF, EDLI and Pension
- Full postal Address of Claimant
This form can be used by both UAN and Non-UAN based employees EPF accounts. Also, if an employee not updated the Aadhaar number to his/her EPF account, then also you can use the same EPF. Hence, this form must be filled by the nominee, legal heir or a guardian (in the case of minor nominee or legal heir).
While submitting composite Claim Form – Death Cases following documents required to enclose.
- Death Certificate of an employee
- Joint photograph of all the claimants
- Date of Birth certificate of children claiming pension
- Scheme Certificate (if applicable)
- Copy of canceled cheque/attested copy of the first page of bank Pass Book
